Erik ten Hag's side made the perfect start after Rasmus Hojlund rifled in the opening goal inside three minutes only for Richarlison to restore parity from a corner.
Marcus Rashford scored his first goal at Old Trafford since May to put United ahead at the break.
However, Rodrigo Bentancur took less than a minute to equalise following the restart to seal a share of the points.
The result ensured United leapfrogged Chelsea up to seventh, but Spurs remain behind rivals Arsenals — outside of the top four on goal difference.
This was the first time Manchester United had failed to win a Premier League game at Old Trafford when leading at half time since 12th February 2022 (1-1 v Southampton), with the Red Devils having won each of the previous 15 when going into the break ahead.
Since beating them twice in the Premier League in 2018 under Mauricio Pochettino (2-0 at home, 3-0 away), Tottenham have only won two of their last 11 games against Manchester United in the competition (D3 L6).
