Bayern coach Thomas Tuchel has kept midfielder Joshua Kimmich at right-back, where he has played the past two league games.
Raphael Guerreiro was preferred at left-back, ahead of Alphonso Davies who returned on Friday after missing several matches with a knee strain.
Tuchel's side have won just one of their past five matches, losing the first leg 1-0 in Rome.
Captain Ciro Immobile, who converted a penalty to give Lazio the edge in the tie, once again leads the attack for the Italians.
Luca Pellegrini has been preferred to Elseid Hysaj in the starting XI, pushing Adam Marusic onto the right.
Matias Vecino was named in midfield ahead of Danilo Cataldi and will line up alongside Matteo Guendouzi.
Guendouzi, Pellegrini and Marusic were all sent off in Friday's 1-0 home loss against AC Milan in Serie A.
